What East Texans need to know about kratom

In most gas stations or smoke shops, you will likely see colorful displays for a supplement called kratom. It often is marketed as a natural energy booster or a safe way to manage pain without a prescription. But providers in East Texas see a different story in clinics. Jasmine Douglas, M.D., addiction medicine specialist, shares important information about this potentially harmful product.

The reality is that while kratom is derived from a tree, its effects on the brain are anything but mild. At low doses, it acts as stimulant, making you feel more energetic. At high doses, it acts as an opioid by attaching to the same receptors in the brain as heroin or prescription painkillers.

A large concern is the rise of concentrated extracts like 7-Hydroxymitragynine (7-OH) that come from the kratom plant. These extracts can be 10 to 50 times stronger than the natural leaf. This isn’t about tea from the kratom leaf anymore; this is about a potent chemical that can quickly lead to addiction.

Texas has taken steps to address this through the Texas Kratom Consumer Protection Act, also known as Senate Bill 497, passed in 2023. This law made synthetic versions of these chemicals illegal and puts a strict 2% cap on the concentration of 7-OH in a product. The law also requires you to be at least 18 to buy it. Even with these protections, the “natural” label can be misleading.

Some people turn to these products to avoid the stigma of traditional pain medications or opioids, but the consequences can be just as severe. A daily habit can cost between $40 and $60, and the withdrawal process often is described as a mix of severe flu and intense panic. In many cases, kratom can be harder to quit than standard prescription opioids.
