Breast augmentation

Dr. Paul Critelli is board-certified in plastic surgery and specializes in breast surgery. Breast augmentation gives women the opportunity to have fuller, shapelier breasts, improve balance of the female figure, as well as enhance self-image and self-confidence. Breast augmentation, also referred to as breast enlargement surgery, is a procedure that places implants within the breasts to make them appear larger. It is often used to restore volume after weight loss or pregnancy. It can help correct asymmetries is breast size.

Ideal candidates for this procedure are those who desire a larger cup size, perky breasts and more breast cleavage. Implants come in silicone shells, which may be filled with either a saline solution or silicone gel, and can vary in shape, size and texture. There may also be cases when patients desire a more natural look with a natural filler and consider a fat transfer instead of artificial implants.

Saline breast implants

Saline implants are an excellent choice for many women deciding what type of implant to use for breast augmentation. Because these implants are filled with a sterile salt water solution, potential leakage poses no threat as the solution is safely absorbed by the body. Saline implants also cost several hundred dollars less than silicone. One of the disadvantages of saline implants is that they are more prone to show rippling, particularly over the lateral breast. This is something to consider especially for women who are thin and have low breast volume tissue. Those who opt for this type of implant must be at least 18 years old.

Silicone breast implants

Silicone breast implants are very popular today and are highly recommended by surgeons for patients who are thin and desire a natural look. Since the FDA validated their safety and efficacy, safety concerns surrounding these implants are today almost no different than that of saline. The silicone shell is prefilled with silicone gel and comes in different sizes, shapes and textures. Unlike saline implants in which leakage is absorbed by the body and implant volume is visibly flat, it is difficult to tell when a silicone implant ruptures. An MRI is required to detect any rupture. Patients considering silicone implants must be at least 22 years old.

Fat transfer

For patients who desire fuller breasts but do not want an artificial implant to achieve this, there is also an option known as a fat transfer. In this procedure, the doctor will harvest fat from the patient’s flank, abdomen and/or thighs via liposuction. The fat collected will be cleansed and strained. The purified fat tissues will then be gradually reinjected within the breasts with smaller syringes so the shape is evenly distributed.

Profiles

Breast implants come in a variety of different profiles and widths. The profile determines how perky or enhanced the breasts’ projection will be. The width of the implant determines how wide the breast will be. Dr. Critelli knows choosing the right profile can sometimes be a difficult decision, and will help guide you in selecting the right implant to meet your goals. Standard profiles come in low, moderate, high and extra high projection. As you increase projection you gain medial breast cleavage and lift the nipple areolar complex. The implant profile and width give you an implant volume.

Shape

The standard shape of a breast implant can either be rounded or tear-shaped. Each shape serves a different aesthetic function as it pertains to the patient’s goals. Rounded implants produce a shapelier curve on the top portion of the breast. The teardrop implants are shaped to copy the natural appearance of the breast, so they will not appear as high.

Implant placement

The two most common placements are on top of the pectoralis muscle or under it. When the implant is placed under the muscle, a pocket is created under pectoralis major muscle. When it is placed above, a pocket is created in front of the muscle and below the breast tissue. There are advantages and disadvantages to each.

Many surgeons prefer the sub-pectoral pocket (behind the muscle). This helps to keep the implant in place for a longer period of time and resists the forces of gravity and time, particularly with larger implants. It is also thought to decrease scar tissue formation or capsular contracture. Placing the implant behind the muscle also provides a natural, smooth, upper pole look that many women prefer.

Sub-glandular implant pocket has several advantages. This pocket is often associated with less postoperative pain and recovery time. Placement can be done under local anesthetic avoiding a general anesthetic. The sub- glandular pocket also given more medial breast cleavage and more lift to the nipple areolar complex. The down side to this pocket is that there is no muscle to support and protect the implant from gravity and forces. Therefore, results tend to last a shorter time with need for correction sooner, especially with larger implants.

Incision type

There are several standard incision approaches for breast implants. The first is known as an inframammary incision, which is an incision near the breast fold. This is by far the most common placement for the scar. The scar can often be placed within the fold making it difficult to see from most views. Second, is an incision made under the areola, which is known as a periareolar incision. This is often used on women who do not have a well-defined fold. A third option is to place the incision within the armpit, known as a transaxillary approach. It is up to the patient’s desires and surgeon recommendation as to what approach is best to take. Scarring is also discussed at this time with each method.

During/after the surgery

Breast augmentation is an outpatient procedure that may last from an hour to several hours. It is performed using general anesthesia at UT Health Tyler. All activities must be limited within 48 hours after surgery. There will be bruising and swelling, and a compression support bra will also be worn to help in healing. It is normal to feel tightness around the chest area as the skin adjusts to the breast’s new shape. Most patients can resume normal activities within 7 days; exercise should be avoided for 4-6 weeks. It takes approximately 6 months for the breasts to look, feel and function normally in regards to softness, sensation and final placement.

FAQs

Implant vs. fat?

This depends on the patient’s breast make up. Those who opt for implants may choose between saline or silicone as each has its own advantages. Generally, patients who have low breast tissue will benefit from silicone implants; otherwise, saline implants are recommended. For fat transfer, the advantage is that it poses less risk since there is no introduction of foreign material in the body. However, the results can change with fluctuations in body weight.

Silicone risk?

Negative notions about silicone started back in the early 90s when there were cases of leakage and complications. With today’s advancements in engineering, the silicone-based shells are made durable and safe, and have been proven so by the FDA. Today, in fact, silicone is more preferred than saline since they last longer and feel more natural. The rates of reported complications have significantly decreased in the last decade.

Implant or lift?

Breast augmentation is a cosmetic procedure designed to enlarge breasts while a breast lift is designed to tighten the loose skin of a droopy breast. Both resolve different breast issues. There are cases that the procedures are simultaneously done when it is agreed that a breast lift with augmentation is necessary.

Silicone leakage?

Today’s silicone implants are strong, durable and safe. Technical advancements have come so far, that even if a silicone implant does rupture, the shell of the implant will keep the silicone gel encased and protected from entering the body. In a rare case that a silicone implant does rupture, an MRI will be needed to detect the rupture. This is the main reason why MRI checks are recommended 3 years after surgery and every 2 years succeeding the first MRI.

Implant replacement?

Implants do not last forever and Dr. Critelli advises his patients to anticipate replacement every 15 years. As long as the implant shows no signs of damage or visible defects, such as breast rippling or a deformity, replacing them is not necessary. What is necessary, however, is a periodic check of the implants to make sure that they are still intact.

Breast augmentation with lift

Breast augmentation with a lift is an option for women who have sagging breasts and require a lift, yet want additional volume with implants. This combined surgical technique incorporates skin tightening, which removes excess skin and lifts the breast to a higher place on the chest wall. While some women have enough breast tissue to end up with a result they are happy with, many prefer to have implants placed within the breast to increase the size. Dr. Paul Critelli, board-certified plastic surgeon at UT Health East Texas Cosmetic Surgery Center will meet with you to discuss your needs and goals, then advise as to whether or not a lift is needed first. It’s important to understand that if a breast is sagging to a certain degree, placing an implant without performing a lift could result in a poor outcome. Breast augmentation with a lift creates beautiful, shapelier breasts for countless women.

About the surgery

The surgery begins with the breast lift before placing the implant. During the procedure, excess skin is trimmed away, the underlying tissues are tightened with sutures and the breast is reshaped. This completes the lift portion of the procedure. Sometimes the areola is also reduced and the nipple repositioned. When the lift is done, the breast implant will be placed either on top of the chest muscle or underneath it based on the patient’s condition.

Breast lift incision

All breast lift procedures will require incisions due to the removal of skin. There are several incision patterns that can be considered:

  • Incision made along the line surrounding the areola;
  • Incision made along the line surrounding the areola plus a vertical incision down to the natural crease below the breast; and,
  • Incision made along the line surrounding the areola plus a vertical incision down to the natural crease below the breast, then another horizontal line following the direction of the fold under the breast.

Breast augmentation

Breast augmentation is a common cosmetic procedure. Patients can choose between a silicone implant and one that is filled with saline. The type of implant chosen depends on the patient’s aesthetic goals and the chest form. Some surgeons prefer to do a breast lift, and then see how the shape is affected before moving forward with implants. There are cases when patients are content with the results of the breast lift and no longer desire to have the augmentation portion.

During/after surgery

This surgery is an outpatient procedure and is performed at UT Health Tyler under general anesthesia. Following the procedure, patients will need to limit movement, especially in the chest area for approximately 48 hours. Because this surgery represents a combination of procedures, the recovery is a bit longer and can take up to 8-10 weeks to recover. Optimal results will be evident within 6-12 months. This is the time period it takes for the implants to settle in their new position and for the scars to heal.

Implants for breast sag?

Implants will surely help increase breast size, but it is a breast lift that resolves the sagging. Breast sag is caused by overstretched skin that has lost its elasticity due to natural conditions such as pregnancy, weight loss or aging. Placing an implant does not correct the breast projection, in fact, it could make it worse. A lift corrects any sagging to create a tighter breast appearance.

Secondary surgery?

Breast lifts can last for years, especially when patients adhere to a healthy lifestyle. Any significant weight gain or additional pregnancies could compromise the results of the surgery. This is why it is recommended to have this type of surgery when the patient is at or near their goal weight and do not plan to become pregnant in the future. As for breast implants, while they may not last a lifetime, they are manufactured to last up to 15 years or longer.

Unsightly scars?

Due to the nature of the procedure, scarring is unavoidable. The incisions made during a breast lift are patterned to best meet the aesthetic goals of the patient. Although these incisions can be concealed under clothing, they will still exist. The good news, is that over time and with proper scar care, they do fade and become barely noticeable. Most women find that their new shape and beautiful figure outweigh the scarring.

Breast lift

A breast lift, or mastopexy, is a cosmetic procedure to correct breast ptosis, also described as a breast that sags and droops downward. This is a natural process often associated with pregnancy, aging, gravity and weight fluctuations. Dr. Critelli is board-certified in plastic surgery and specializes in breast surgery at UT Health East Texas Cosmetic Surgery Center. He uses techniques that remove and tighten excess skin of the breast so that the breast and nipple project naturally forward and sit higher on the chest wall. This restores a youthful, perky breast appearance, while incisions are positioned to optimize healing and hide scars within natural folds and contours. Augmentation using an implant may also be combined with this surgery according to patient’s aesthetic goals.

About the surgery

During a breast lift surgery, incisions will be made around the skin-color line of the areola. It may extend vertically, down to the crease of the breast, or extend further to a horizontal incision following the breast crease direction. How extensive the incision will be depends on how much skin must be removed. The excess skin will be trimmed away, and the underlying tissues tightened. The remaining skin will be stretched up and closed with sutures. An implant placed underneath the chest muscle may also supplement the lift as it can provide additional support in keeping the improved projection in place.

Incision types

Women who contemplate undergoing breast lift surgery often wonder what the incisions will look like and how bad the scarring will be after the procedure. The incisions vary depending on how severe the breast sagging is and how much tissue volume is present. The most common incisions are noted below.

  • Incision made around the areola border
  • Incision made around the areola, plus a vertical incision down to the natural crease below the breast
  • Incision made the around the areola, plus a vertical incision down to the natural crease below the breast, and another horizontal line following the direction of the breast fold.

During/after surgery

Breast lift surgery is performed using general anesthesia at UT Health Tyler, an accredited surgery center. On average, the procedure may last up to 2 hours. The recovery will vary as some patients will have had a breast lift in combination with a reduction surgery, or with augmentation using implants. Regardless, patients will be encouraged to move around within the first 48 hours. Activities such as carrying heavy objects or any activity that puts pressure on the chest must be avoided. Most patients can resume normal activities within a couple of weeks, but exercise should not be attempted for 6-8 weeks. Scars are part of the process, but with extra care the marks will heal and fade, and many patients feel they become secondary to a more beautiful breast appearance.

Expected cost

Insurance does not cover breast lift surgery as it is considered cosmetic in nature. There are few exceptions depending on the patient’s special circumstances, so it is best to confirm with the insurance coordinator if any coverage is valid. Lift or augmentation?

It depends on the patient’s aesthetic goals and the current breast form. A breast lift will help correct a sagging, drooping breast. Many times after this surgery, patients are happy with their breast size because the lift alone creates a fuller breast. If the patient wishes to have more volume and increase the breast size, implants will resolve this issue. Before surgery, each patient will have a thorough assessment and consultation with Dr. Critelli. During this appointment, Dr. Critelli will discuss the procedure details and if a lift alone is all that is needed, or if the patient will need a combination surgery with an augmentation as well.

Nipple repositioning?

It is common in a breast lift procedure that the nipple is repositioned so that it looks proportional to the newly shaped breast. When excess skin is trimmed away, a portion of an overly stretched areola (the colored skin around the nipple) is simultaneously trimmed away. The goal is for the nipple and areola to be within a normal size and position.

How bad is scarring?

When incisions are made to trim away the skin, scars are an inevitable part of the process. Dr. Critelli is an expert when it comes to minimizing scarring with precise incisions and suturing techniques. He also uses advanced incision site dressings to minimize tension. This along with a post-surgery scar care program will provide optimal healing and scaring.

Breast reconstruction

Dr. Paul Critelli, board-certified plastic surgeon at UT Health East Texas Cosmetic Surgery Center works with patients throughout East Texas and the DFW area who have had a mastectomy, or removal of their breasts. This may be part of breast cancer treatment, or due to other medical reasons. These patients may opt to have breast reconstruction to restore a normal appearance. There are a number of reconstruction techniques to consider, so it is important to find a surgeon who is adept at them all so that you get the right procedure for you.

Dr. Critelli will meet with you early in your treatment planning to discuss your reconstructive options. He will then work with you to develop a customized plan to meet your needs and goals. Surgery may be done at the same time as the mastectomy, or it may be done later as a separate sessions, known as delayed reconstruction. In coordination with the UT Health East Texas HOPE Cancer Center, Dr. Critelli works with some of the best cancer surgeons in the region to optimize your cancer treatment and reconstruction, producing unparalleled results.

Things to consider

The surgical planning for a breast reconstruction typically begins at the same time as the planning for the mastectomy. There are several elements that Dr. Critelli will discuss with you that determine reconstructive options, timing, and overall cosmetic appearance of the reconstructed breasts.

  • Orientation of scars on the breasts. Dr. Critelli makes every effort to minimize visible scars on the breasts. While a scar is required to remove the breast its placement can have a dramatic effect on the final appearance of the breast. Some patients are candidates for a “no scar “mastectomy where the scar is hidden in the breast fold and is often imperceptible. If this is not an option, reconstruction can be done through a small vertical incision.
  • Nipple preservation. This can often be used with similar oncologic results in patients with small tumors, away from the nipple. The nipple is a unique structure that makes the breast look like a breast and is difficult to recreate this once lost. Dr. Critelli has several techniques to preserve the nipple safely.
  • Breast Shape. For some women who have a very favorable breast shape to begin with this can often be preserved, so that the reconstruction looks very similar to the breast prior to mastectomy. For others, particularly after children, breast feeding, and/or significant weight loss, sagging of the breasts needs to be addressed as well. Dr. Critelli has several techniques to lift the breasts at the time of breast removal. This significantly improves the end cosmetic result.
  • Radiation Therapy. This will be extensively discussed in your consultation, because radiation adds an increased layer of complexity to the reconstruction.
  • Symmetry procedure. The final step in reconstruction is to match the other native breast to the reconstructed breasts size and shape.

In general, there are four phases to breast reconstruction.

  • Recreate breast volume and shape.
  • Improve shape, contour and exchange temporary implants for permanent implants. It also often involves use of liposuction to harvest fat that is then be grafted to the new breasts.
  • Nipple and areolar reconstruction.
  • Symmetry procedures including, breast lift, augmentation or reduction to the other, native breast.

In general the first stage is the longest surgery with the most recovery time and the subsequent stages are minor, outpatient procedures. The stages can sometimes be combines to shorten the reconstructive course. The general goal is to help the patient regain a normal appearance of the breasts as part of the final phase of breast cancer treatment.

About the surgery

Surgical techniques vary and today, patients are getting more options that can match their personal preferences. It is important to find a highly specialized plastic surgeon to perform breast reconstruction and one who will work with the cancer surgeon to achieve a natural result.

Reconstruction with implants

Breast implants are the most common method of breast reconstruction. When implants are being considered, the reconstruction process is usually performed in a multi-stage treatment session. A tissue expander is placed in between the chest muscle and the skin during the mastectomy. This expander will be gradually filled with a saline solution to expand the skin until you reach your desired size and shape. It is then exchanged for a permanent implant. These are the same implants used in cosmetic breast augmentation and can either be saline or silicone. Sometimes, the saline-filled expander is retained instead of an implant.

Autologous reconstruction

Flap procedures harvest skin, fat and muscle tissue from donor sites and transfer it to the breast. The surgeon may choose between one of the following common techniques:

  • TRAM flap involves grafting skin and muscle tissues on the lower part of the abdomen.
  • DIEP and SIEA flap graft tissue from the lower part of the abdomen, but harvests only the skin and fat tissues instead of muscles.
  • Latissimus Dorsi flap uses skin and muscle tissues from the upper back.
  • DUG flap harvests skin, fat and muscle tissues from the inner thighs near the buttocks area.

Nipple/areola restoration

When a breast is surgically removed, the nipple and areola may need to be sacrificed. To recreate this unique structure, skin and tissues are formed to in a three dimensional, projecting, nipple while the areola is achieved by skin grafts or tattooing. This is done after the breast reconstruction has fully healed.

During/after surgery

Breast reconstruction uses general anesthesia, and the patient is often required to stay in a hospital if done simultaneously with the mastectomy. Patients who have flap surgeries sometimes stay up to 5 days for monitoring. There will be discomfort and pain within the first week, and the patient may feel disoriented with the newly formed chest. Temporary drain tubes and bandages are placed to assist in healing. It can be unnerving to go through a major body change; however, patients find it gratifying and relieving that they are able to once again look and feel normal as they start their new journey towards becoming a cancer survivor.

Expected cost

While most cosmetic procedures are not covered by insurance, breast reconstruction as part of cancer treatment is usually covered. The Women’s Health and Cancer Rights Act of 1997 allows the recovering cancer patient to undergo the surgery and apply insurance coverage towards the total cost. Which surgeon?

With breast reconstruction, a plastic surgeon will work alongside the cancer surgeon and be primarily involved and responsible as it pertains to restoring the appearance. It is a highly specialized surgery so it is critical to choose a plastic surgeon that has years of successful experiences in this type of surgery.

Delayed reconstruction?

If a patient knows she will want to restore her breasts with a reconstruction procedure, it is recommended that this process begins at the time as the mastectomy. During the breast removal portion, the tissue expanders can be put into place, which eliminates a new surgery. This ultimately decreases the risk for complications and improves the overall cosmetic result.

Breast sensation?

Because the breast is completely removed and a new one is formed, there will be a loss of sensation. Over time, there is a possibility that this sensation can be slowly regained. This is different for every patient. While some patients regain their sensitivity, there is no guarantee that this will be the case for everyone.

Breast reduction

People who have larger than desired breasts may consider a breast reduction procedure to reduce their breast size and improve overall breast shape. This is a very common surgical procedure that Dr. Critelli specializes in. It is not uncommon for people to find their large breasts interfering with everyday activities. People with large and heavy breasts often feel self-conscious, find difficulty in doing physical activities such as exercising and complain of symptoms like back pain, skin irritations and painful shoulder grooving from tight bra straps. It can also cause headaches and numbness in the fingers.

Breast reduction removes excess tissue makes the breasts lighter, lifts and reshapes the breasts to improve shape and body proportions. The most common comment from patients I see after surgery is, “I wish I would have done this sooner.” There is no reason to go through life in discomfort.

About the surgery

It is important to find a highly skilled surgeon who has experience with this procedure. Dr. Critelli uses the latest surgical techniques to create a breast with improved shape, nipple position and cleavage, while minimizing and hiding surgical scars. It is important to maintain a good blood supply to the nipple areola complex as there is risk of discoloration and loss of sensation when blood flow is restricted. Liposuction is also implemented to aid in excess fat removal, while the loose skin is excised and tightened to form the newly shaped breasts. Asymmetry in breast size, as well as overly large areolas can also be addressed with this procedure.

Incision placement

The most common incision for breast reduction is what they call “the anchor” or a vertical T. With this kind of approach, the incision is made around the areola, extends vertically down to the breast crease, and then is followed by another horizontal incision along the direction of the fold. The areola will be repositioned once the breasts have been resized and reshaped. A lift will also be done to allow the breasts to be placed higher on the chest.

During/after surgery

Breast reduction is done at UT Health Tyler, an accredited surgical center, under general anesthesia as an outpatient procedure. The surgery usually takes two to four hours. There are no drains. For the first several weeks, the surgeon will recommend the use of a compression bra to protect the breasts. Post-operative pain is usually mild. Most patients can go back to work within seven to ten days and may resume physical activities, such as exercise, between six to eight weeks post-operative. Results following a breast reduction are usually visible immediately after the surgery. Nipple sensitivity will gradually return within the first year.

Expected cost

Insurance does not normally cover cosmetic procedures. However, patients with larger breasts have tendencies to experience physical problems, which can provide medical health conditions of which insurance may cover. Will results last?

Breast reduction is a long-term solution for patients who are affected with excessively large breasts. In cases where there is significant weight gain, the results may be compromised just like any cosmetic procedure that modifies body shape. Natural aging and gravity will take their toll over time and will still affect the breasts with or without the reduction. It is important to maintain a good health routine so that the enhancement lasts for a long time.

Smaller areola?

For women with larger breasts, the areola is also excessively large. In a breast reduction surgery, an areola reduction is also considered. As an incision is made surrounding the areola, the excess skin will also be trimmed so that it is proportional to the breasts’ new shape.

What size can I be?

This varies depending on the doctor’s initial evaluation and the patient’s preference, but often the patient can choose their desired size. The reduction can vary from one or two cup sizes smaller, up to more than half the breast volume.

Blepharoplasty

To address the excess, often sagging upper or lower eyelid skin and “bags” around the eyes, upper or lower blepharoplasty may be recommended. The excess skin of the upper eyelids may be significant enough to impair a person’s vision or weigh down their eyelids enough to cause eye fatigue. In these cases, the problem goes beyond cosmetic to functional and insurance may sometimes cover upper blepharoplasty as a functional procedure. Both upper and lower blepharoplasty can improve the tired, worn out, aged appearance of the eyes.

During/after surgery

Blepharoplasty, or eyelid surgery is performed with general anesthesia at UT Health Tyler. If the procedure involves both upper and lower eyelids, the surgeon will typically manage the upper eyelid first. On average, it takes one to two hours to complete the operation. Permanent results?

The results of liposuction can be seen for many years following surgery if the patient follows a healthy diet and exercise program. Removed fat cells do not come back, but patients can still gain weight if their remaining fat cells grow.

Too much fat?

Yes, there can be too much fat. The American Society of Plastic Surgeons does not recommend removing more than 10 pounds of fat for the safety of the patient. Lipo or abdominoplasty?

Tummy tucks are reserved for patients who have stretched, loose skin in the lower abdomen as a result of pregnancy or obesity. Liposuction, on the other hand, is for those who want to remove excess fat in the abdomen but still have a healthy skin tone. During the initial consultation, we will provide a thorough assessment and determine if liposuction alone is all that is needed, or if a tummy tuck will provide the best results.

Combination procedures?

Normally, abdominoplasty is performed to improve loose skin and excess fat around the abdomen area. However, sometimes other cosmetic procedures, such as breast augmentation or a breast lift are done to further improve the overall look and feel of the body. When this combination type surgery exists, it is known as a “mommy makeover”. It is very popular among women after pregnancy who wish to get their pre-pregnancy body and shape back to normal.

How bad are the scars?

Unfortunately, with a tummy tuck surgery, scars are unavoidable. However, extra measures are taken to make sure that scarring is minimal. With proper technique and minimal tension of the skin layer scars can heal favorably. The incision line is placed in areas that can be concealed by undergarments. Previous C-section scars will be incised in the same spot for the tummy tuck. During the healing process, we will provide a post-operative protocol that includes scar treatment to help assist the scars in becoming less visible.

Pregnancy after tummy tuck?

It is our recommendation that women who decide to have more children delay a tummy tuck procedure. The stretching of the skin as a result of another pregnancy can compromise the results of this procedure.
