Home Telemonitoring

People facing chronic health issues can remain confident living at home with home telemonitoring, a system that connects patients and their healthcare team on a daily basis. Telemonitoring monitors vital signs and symptoms and transfers the data directly to the UT Health East Texas Home Health office, where care and treatment plans are discussed with a physician and adapted accordingly.

The telemonitoring system decreases hospitalization and improves the quality of life for people with

  • Heart failure
  • High blood pressure
  • Depression
  • Weight problems
  • Chronic obstructive pulmonary disease
  • Post-traumatic stress disorder (PTSD)
  • Dementia
  • End-of-life issues
  • Other complex conditions
